When you use the file->open, you need to navigate to where you saved the .opt file in PS. You need to know which folder you saved it to. I recommend creating a folder in which to save .opt files, so you don't have to separate them out from sib files in a list! Sib will automatically identify .opt as well as .sib and .mid files.
On re-reading your OP, I am not clear on why turning the page upside down made a difference. It should still find the staves on the page, but the music will be upside down and effectively produce nonsense if you try to recognise it.
PS scans a page and then automatically identifies staves on the page, marking them with purple staves to show what it has recognised.
If you give it a low quality original or if your parameters are wrong for the original size of the page, then these purple staves may not be accurate. It is usually pretty good at knowing when the staff and the purple markers are not aligned.
If you look at the screen, you should be able to tell if the music is the right way up.
